Job Overview

We are seeking a highly skilled professional to provide culturally safe and trauma-informed case management services to Aboriginal women in a residential program. As a key member of our team, you will deliver meaningful care to clients with a small caseload, promoting relapse prevention, harm reduction, and overall wellbeing.

Key Responsibilities:
  • Provide individualized case management services based on client needs and goals
  • Support clients in achieving their objectives through goal-setting and planning
  • Build strong relationships with clients and their families to ensure effective support and service delivery
  • Maintain accurate records and case notes to track client progress and outcomes
Requirements:
  • Completion of a Certificate IV in Community Services (AOD) or related qualification is preferred
  • Experience in case management within AOD, therapeutic, or community settings is essential
  • Strong communication and cultural awareness skills are critical for success in this role
  • A driver's licence, Ochre Card, and Police Check are also required
