Job Title: Person-Centred Care Specialist

Job Overview:

\

As a dedicated member of our team, you will provide person-centred care to individuals with psychosocial disabilities. Your role will involve assisting participants in expanding their experiences and interests, empowering them to grow and develop their skills.

\

Key Responsibilities:

\
  • Provide quality support to assist participants with daily living activities, including personal care, meal preparation, and household chores.\
  • Support participants to grow in independence and skill while enabling them to self-advocate.\
  • Foster positive relationships between all stakeholders, including participants, family and friends, colleagues and other professionals.\
  • Implement Individual Support Plans and Behaviour Support Plans.\
  • Ensure a safe and healthy environment for participants, including managing medication and administering first aid when necessary.\
  • Work flexible hours, including a willingness to work within a 24/7 rotating roster.\
  • Provide timely progress reports, shift notes, incident reports and other relevant documentation.

Essential Requirements:

\
  • Previous experience working as a Psychosocial Support Worker, Mental Health Support Worker, Disability Support Worker or Youth Worker.\
  • Experience in supporting participants with potential concerning or challenging behaviours.\
  • An understanding of the impacts of mental health.\
  • Commitment to implementing and following support plans.\
  • Excellent communication, both written and verbal.\
  • Proven ability to work as a part of a close-knit team.\
  • Genuine desire to engage with participants in a way that promotes positive outcomes.

Qualifications and Checks:

  • Certificate III (desired but not compulsory).\
  • Full NSW Driver's License.\
  • Working With Children Check.\
  • NDIS Worker Check (at your own cost).\
  • First Aid (less than 3 years old).\
  • CPR (less than 12 months old).\
  • Australian Citizen or Permanent Resident.
