The primary purpose of the Case Manager – Young Women's Support is to assist with the immediate and ongoing support needs of young women or those that identify as female (16-25), that are at risk of or experiencing homelessness predominately in the Southeast of Melbourne.
This primarily consists of providing a range of flexible support services and high-level coordination to clients, through a strength based and trauma informed, case management support model to maximize independence and minimize service dependence.
Young Women's Support Service acknowledges and is designed to address the unique needs of young women at risk of entering or already linked into the youth homelessness system.
Using outreach and meeting young women where they are in the community, with a focus on housing and its stability, connection to community, support of multiple needs including but not limited to mental illness, substance abuse issues, engaging in education or employment and general wellbeing for support periods of up to 3 months.
The purpose of this role is to support young women and those who identify as female. As such, only candidates who are women or who identify as female will be considered.
This role is permanent, part-time (68.4 hours per fortnight), based in Cheltenham.
This is a Targeted position and therefore open to applicants from an Australian Aboriginal and/or Torres Strait Islander heritage by an exemption granted under section 12 of the Vic – Equal Opportunity Act 2010. This is a recruitment strategy to increase the Aboriginal workforce.
What you'll be doing- Conducting comprehensive assessments of clients' needs in order to develop collaborative case plans.
- Approaching and building a comprehensive network of external service providers across metro and surrounding suburbs of Melbourne in order to receive and complete direct referrals.
- Providing a comprehensive case management service to clients, including assistance with accommodation, advocacy, assessment, referral, and the development of community supports.
- Assisting clients to maximise their options and potential by facilitating their involvement in community and social activities, recreational and educational or training opportunities.
- Advocating on behalf of clients where agreed, including liaising with mental health, drug and alcohol services, legal institutes, government, and non-government service providers.
- Initiating/attending case management meetings with professionals involved with clients ensuring collaborative service delivery.
- Demonstrated experience in client facing human services, social work or other relevant discipline, with a preference for an understanding of youth housing and homelessness policy, context and systems.
- Tertiary qualifications in Social Work, Psychology or a related discipline highly desirable.
- Experience in supporting people with complex challenges and support needs.
- Understanding of trauma-informed practice and the individual and structural causes which lead to addiction and dependence.
- High degree of initiative and analytical skills, including well-developed organisational and time-management skills.
- Demonstrated capability in verbal and written communication.
- Excellent interpersonal skills.
- Demonstrated capacity to maintain a high level of confidentiality.
- Demonstrated capacity to complete tasks and effective time management.
- Ability to work collaboratively with other organisations to achieve client outcome.
- A valid Victorian driver's licence (mandatory).
- Understanding of, or a demonstrated commitment to upholding, Child Safe Standards in the workplace.
- Strong communication and interpersonal skills, with the ability to engage effectively with people from diverse backgrounds.

Salary: 45.92 - 49.41 an hour + Superannuation + NFP Salary Packaging (tax savings) + Additional Wellbeing Leave Available
