Role Summary

We are seeking a dedicated professional to join our supportive housing program, working in partnership with VincentCare and Sacred Heart Mission through a multi-disciplinary team that includes supportive housing staff, mental health clinicians and peer support settlement staff.

This is a permanent full-time position located in North Melbourne, VIC. The role provides assertive engagement, brief intervention, case management and service coordination with people who have experienced chronic homelessness and/or primary homelessness.

The focus of the role is to ensure that the service provided is person-centred, effective in achieving client goals and program outcomes.

Key responsibilities include:

  • Assertive engagement with clients to understand their needs and circumstances
  • Development and implementation of individualised case plans to achieve client goals
  • Collaboration with internal and external stakeholders to ensure effective service delivery
  • Monitoring and evaluation of client progress and service outcomes

Required Skills and Qualifications

The ideal candidate will possess:

  • Relevant degree or above qualifications
  • Advanced communication, written and organisational skills
  • Ability to meet tight deadlines and ensure service delivery outcomes meet required targets
  • Initiative to solve service delivery challenges and ability to manage crisis situations
