About the Role
This exciting opportunity is available for a skilled professional to work in our Community Programs team in South Western Sydney on a full-time basis. The role may require travel between or working at other sites as needed.
The successful applicant will conduct assessments, facilitate educational groups, provide face-to-face and online counseling, and make referrals to other services as required. They will assist with creating linkages to support services to enhance recovery outcomes for clients.
The Intake Specialist duties include providing care coordination, contributing to educational programs focused on AOD, MH, and social emotional support, and evaluating client treatment plans. Responsibilities involve maintaining client notes and data, and liaising with other agencies and support services.
Key Requirements:
- Relevant VET qualification
- Demonstrable experience supporting persons with AOD support needs, Mental Health support needs, and co-existing support needs
- Experience working in a team environment to deliver coordinated support
- Experience undertaking brief and comprehensive assessment and ongoing assessment of client support needs
- Computer literacy including understanding of MS Office Suite and use of Client Management Systems
- Demonstrable communication skills including report writing and case note writing
- Understanding of the principles of the Privacy Act and its implementation in service delivery
- Knowledge of evidence-based treatment, harm minimisation, care coordination, and support strategies
