Empowering Lives: Disability Support Role

We are seeking a skilled Disability Support Specialist to join our team and contribute to the empowerment of individuals with disabilities.
This role involves working closely with clients, their families, and other stakeholders to establish strong relationships and ensure seamless coordination of support services.
- Strengthen and enhance client's ability to coordinate supports and participate in the community
- Regularly monitor client's NDIS plan to ensure services are being delivered within allocated budget
- Ensure mainstream services meet their service obligations to clients (i.e. housing, education, justice, health)
- Provide regular reports on outcomes and success indicators to the client and NDIA
Key qualifications for this role include:
- Demonstrated experience in working with people with disability and/or mental health issues
- Ability to work in a KPI-driven environment where motivation and commitment are essential
- Excellent verbal communication skills and demonstrated ability to engage with clients and their families to support them as participants of the NDIS
- Strong written and computer skills for creation of plans, programs reports, correspondence and other documents
- Ability to work autonomously and use initiative
The ideal candidate will possess excellent interpersonal skills, be highly organized, and have a passion for making a difference in the lives of individuals with disabilities.
