Job Title: Community Case Manager
About the JobThis role undertakes assessments and briefs to individuals with complex needs, aiming to support their successful transition back into the community.
The position is based in prisons and works closely with court-based programs. Regular travel to different prison and court locations is required.
Key Responsibilities:- Conduct comprehensive assessments of clients with complex needs
- Develop and implement post-release case plans to support clients' rehabilitation
- Collaborate with court-based programs to provide a seamless service to clients
- Travel to various prison and court locations as needed
To be successful in this role, you will have:
- Extensive experience in assessing and supporting clients with complex needs
- Demonstrated capacity to deliver culturally responsive and inclusive services to clients
- Substantial knowledge of treatment and support options for clients with complex needs
- A tertiary qualification related to casework practice, such as social work, psychology, or welfare, or equivalent experience
