Community Case Manager

Our organization contributes to community safety by reducing reoffending through evidence-based practices and connecting individuals with programs, services, and the wider community. We provide case management for people on court orders, those serving post-sentence supervision, and prisoners on parole.
Key Responsibilities:
- Conducting thorough risk assessments to inform our approach
- Preparing comprehensive case plans that address individual needs
- Delivering effective interventions to support positive change
- Coordinating with service providers and care teams to ensure seamless support
- Addressing the underlying causes of offending behavior
- Leading and developing a team of case management professionals to achieve excellence

- Demonstrated expertise in effective case management practices with complex clients
- Excellent verbal communication skills and confidence in managing challenging behaviors
- A proven ability to model pro-social behaviors and inspire positive change
- A passion for supporting individuals within the justice system while contributing to community safety
- An ability to lead, mentor, and develop staff to reach their full potential
