Compassionate Community Care Coordinator

Key Responsibilities:

  • Provide high-quality, patient-centred care that supports individuals transitioning from hospital to home.
  • Develop and implement effective discharge planning strategies that ensure safe, timely, and effective transition of care.
  • Collaborate with healthcare providers to ensure seamless communication and coordination of care.
  • Utilize virtual technologies to deliver responsive, evidence-based care directly into the community.

Requirements:

  • Current Authority to Practice as a Registered Nurse with the Australian Health Practitioner Regulation Agency (AHPRA).
  • Strong passion for providing patient-centred care and improving health outcomes through innovative virtual care solutions.
  • Excellent communication and interpersonal skills, with ability to build strong relationships with patients, families, and healthcare providers.
  • Australian Drivers Licence and ability to travel across the Central Coast.
