This position involves delivering patient-centered care that supports individuals transitioning from hospital to home. As a skilled healthcare professional, you will utilize your clinical expertise and adaptability to implement strategies promoting recovery, independence, and overall wellbeing.
This role requires collaboration with patients, families, and a multidisciplinary team to assess needs, develop care plans, and implement evidence-based practices. You will also focus on health promotion, early intervention, and continuity of care.
About the Position:
- You must hold a current Authority to Practice as a Registered Nurse with AHPRA.
- A strong passion for providing patient-centered care is essential, along with a commitment to improving health outcomes through innovative virtual care solutions.
- Excellent communication skills are required to build strong relationships with patients, families, and healthcare providers.
- You must have a valid Australian Drivers Licence and be able to travel across the Central Coast.
- Being highly organized and adaptable is crucial in this role, as you will effectively use virtual technologies and evidence-based practices.
