\About the Role:We deliver impactful, patient-centered support that enhances independence, access, development, health and wellbeing, and function of our participants.
As a Community Care Specialist, you will work collaboratively in community and clinic settings to make a meaningful difference in your local community.
You'll bring expertise, maturity, and strong time management and organisational skills to the role. Responsibilities include:
- Managing an allocated caseload and delivering timely interventions
- Conducting assessments to evaluate participants' developmental, cognitive, emotional, psychological and physical function and capacity
- Interpreting findings to formulate tailored treatment goals unique to each participant
- Providing evidence-based interventions to enhance their quality of life and meet their goals
- Mentoring and nurturing early career professionals
- Maintaining awareness of current legislation to ensure compliance

Requirements:To be successful in this role, you will have:
- Current registration with AHPRA as a Community Care Specialist
- Excellent time management and organisational skills to manage a varied caseload
- A proactive attitude and interest in continually developing your skills
- Experience in mentoring and supervising other Community Care Specialists
- A genuine passion for working in the disability sector
