Community Care Specialist

\About the Role:

We deliver impactful, patient-centered support that enhances independence, access, development, health and wellbeing, and function of our participants.

As a Community Care Specialist, you will work collaboratively in community and clinic settings to make a meaningful difference in your local community.

You'll bring expertise, maturity, and strong time management and organisational skills to the role. Responsibilities include:

  • Managing an allocated caseload and delivering timely interventions
  • Conducting assessments to evaluate participants' developmental, cognitive, emotional, psychological and physical function and capacity
  • Interpreting findings to formulate tailored treatment goals unique to each participant
  • Providing evidence-based interventions to enhance their quality of life and meet their goals
  • Mentoring and nurturing early career professionals
  • Maintaining awareness of current legislation to ensure compliance

Requirements:

To be successful in this role, you will have:

  • Current registration with AHPRA as a Community Care Specialist
  • Excellent time management and organisational skills to manage a varied caseload
  • A proactive attitude and interest in continually developing your skills
  • Experience in mentoring and supervising other Community Care Specialists
  • A genuine passion for working in the disability sector
