Virtual Care Nurse Opportunity

About the Role:

This is an exciting opportunity to join our Virtual Care Team as a skilled and compassionate nurse. As a forward-thinking professional, you will play a vital role in improving patient outcomes, supporting timely discharges, and preventing unnecessary hospital admissions.

You will work collaboratively with patients, families, and a multidisciplinary team to assess needs, develop care plans, and implement strategies that promote recovery, independence, and overall wellbeing. With a focus on health promotion, early intervention, and continuity of care, your contribution will make a meaningful difference to the lives of those you support.

Key Responsibilities:

  • Deliver high-quality, patient-centred care that supports individuals transitioning from hospital to home, and helps prevent avoidable hospital admissions.
  • Work collaboratively with patients, families, and a multidisciplinary team to assess needs, develop care plans, and implement strategies that promote recovery, independence, and overall wellbeing.
  • Use virtual technologies to deliver responsive, evidence-based care directly into the community.

Requirements:

  • CURRENT Authority to Practice as a Registered Nurse with the Australian Health Practitioner Regulation Agency (AHPRA).
  • PASSION for providing patient-centred care, with a strong commitment to improving health outcomes through innovative virtual care solutions.
  • SKILLED COMMUNICATOR, able to build strong relationships with patients, families, and healthcare providers to ensure safe, timely, and effective discharge planning.
  • Australian Drivers Licence with the ability to travel across the Central Coast.
  • Highly organised and adaptable, with the ability to effectively use virtual technologies and evidence-based practices to support patients and manage complex care in a virtual environment.
