Pharmacist - Medication Safety Specialist

We are seeking a highly motivated and experienced pharmacist to join our team as a Medication Safety Pharmacist. This is a fixed-term, part-time position until 30 June 2025, replacing the current Senior Pharmacist - Medication Safety, who is being seconded to another position.
About the RoleThe successful applicant will work collaboratively with the Lead Pharmacist - Medication Safety to co-ordinate the medication safety program across Grampians Health, a large and diverse multi-campus regional health service. The position requires a whole of Grampians Health focus, and the successful applicant can choose any Grampians Health campus as their primary work location.
Key Responsibilities- Develop and implement system-based, consumer-focused medication safety quality improvement strategies to reduce the risk of harm to patients and enhance medication safety
- Monitor and evaluate medication safety systems through audit and incident report investigation, analysis and reporting
- Provide medication safety related training and education to clinicians and consumers, as required
- Currently hold general registration as a pharmacist with the Australian Health Practitioner Regulation Agency (AHPRA)
- At least 5 years hospital pharmacy experience, including broad exposure to the different aspects of the medication management pathway
- Demonstrated ability to effectively engage stakeholders to achieve desired outcomes
- Demonstrated ability to effectively plan, organise, prioritise and manage time with ability to deliver outcomes to agreed timelines
