Homeless Support Coordinator

Job Summary:

We are seeking a highly skilled and compassionate Homeless Support Coordinator to join our team. In this critical role, you will provide vital support services to individuals, couples, and families who are homeless or at risk of becoming homeless.

Key Responsibilities:

  • Deliver case management support tailored to the needs of program participants, focusing on early intervention strategies to engage and motivate individuals in achieving both program and personal goals.
  • Conduct thorough assessments to determine program eligibility, as well as ongoing evaluations using the Outcome Star tool to track progress and identify barriers to achieving objectives outlined in individualized support plans and review sessions.
  • Ensure accurate and timely recording and maintenance of participant information within electronic data systems, adhering to service agreements, legislation, and organizational policies and procedures.
  • Cultivate and maintain partnerships with relevant community organizations through networking and active promotion of Centacare NQ's initiatives, facilitating referrals and collaborations that enhance the delivery of services.
  • Attend and actively participate in meetings, professional supervision, and continuing education opportunities to stay abreast of industry developments and best practices.

Requirements:

  • A degree-level qualification in Social Work, Human Services, or Behavioural Sciences, or a Certificate level 4 or higher in the community sector, combined with at least two years of experience working within a case management framework in a relevant field.
  • Excellent written and verbal communication skills, as well as strong analytical capabilities.
  • A valid driver's license and ability to hold, or obtain prior to commencement, a Working with Children Blue Card check and Disability Services Yellow Card.
