Mental health clinicians play a vital role in supporting individuals who are homeless or at-risk of becoming homeless. This position requires providing comprehensive assessments, coordination, and clinical interventions tailored to address the mental health and substance use challenges faced by these individuals.
Main Responsibilities- Clinical Assessments: Conduct thorough clinical assessments and develop individualized care plans in collaboration with multidisciplinary teams.
- Assertive Outreach: Utilize assertive outreach methods to engage with homeless/at-risk populations and support stability and access to services.
- Patient-Centered Care: Collaborate with teams to develop and implement person-centered care plans that address mental health and substance use challenges.
- Educational Background: Relevant tertiary degree qualification and appropriate registration/licence.
- Driving Requirements: C Class Motor Vehicle Licence.
- Experience: Significant clinical experience prior to appointment.
- Screening Process: Pre-employment screening, including Criminal History checks.
- Health Clearance: Evidence of vaccination or immunity to Hepatitis A/B, Measles, Mumps, Rubella, Varicella, Pertussis, and Tuberculosis may be required.
