We are seeking a skilled Team Leader to oversee the implementation of individualized supports within supported independent living settings.

You will provide direct support and lead your team across a range of customer needs, including domestic and personal care assistance, as well as helping customers access and positively connect with the community.

This is a part-time permanent position that requires strong leadership and management experience, resilience, and the ability to work independently and as part of a team.

To be successful in this role, you will have previous experience in the health sector, preferably in supported employment or day programs, and hold a Cert 4 or higher in a related field.

Key Responsibilities
  • Lead and manage a team of Support Workers to deliver high-quality, person-centered care.
  • Implement and monitor individualized support plans to ensure customer needs are met.
  • Develop and maintain positive relationships with customers, families, and other stakeholders.

Requirements: Previous experience in the health sector; leadership and management experience; Cert 4 or higher in a related field; ability to obtain NDIS Worker Screen Check and paid NSW and VIC Working with Children Checks.
