We are a leading provider to the National Disability Insurance Scheme (NDIS), supporting over 10,000 participants every year with the care they need, when and where it is needed.

Job Overview

In this role you will assist individuals with complex needs through assessment and person-centred planning. You will also develop and deliver evidence-based support plans within a dynamic support environment.

This role requires collaboration with multidisciplinary teams, including mentoring less experienced professionals.

Key Responsibilities
  • Evaluate individual needs and develop meaningful goals using a positive behaviour support framework.
  • Develop practical strategies to eliminate restrictive practices in home and community settings.
  • Ensure individual plans reflect participants' goals, passions and strengths.
Requirements

To succeed in this role you will have:

  • A degree-level qualification as an allied health professional.
  • Excellent time management and organisational skills to manage a varied caseload.
  • Proactive attitude, motivated by improvement and the opportunity to test and learn from new ideas.
