Opportunity Overview:

We are seeking a skilled Clinical Nurse Specialist to lead our Hospital in the Home team. This position involves providing safe, patient-centred care for individuals transitioning from hospital to home.

About the Position:

  • Lead and motivate a dedicated team to deliver high-quality patient care.
  • Collaborate with multidisciplinary teams to ensure smooth transitions and timely access to care across the healthcare journey.
  • Champion evidence-based practice, quality improvement, and service development initiatives to enhance patient outcomes.
  • Apply advanced clinical expertise to provide guidance, education, and support to staff, patients, families, and carers.

Requirements:

  • A Registered Nurse (Division 1) with current AHPRA registration and relevant postgraduate qualifications.
  • At least 3 years' experience in acute, post-acute, or Hospital in the Home care, with confidence in applying advanced nursing skills in complex situations.
  • Passion for delivering safe, patient-centred care, particularly for individuals transitioning from hospital to home.
  • Clinical leadership, mentoring, and supporting other nurses while promoting professional growth.
  • A current NSW Driver's Licence and ability to work in both hospital and community settings.
