At Concept Care, we're seeking skilled professionals to join our award-winning team of Disability Support Workers. We provide high-quality, person-centered support to clients with complex care, complex behaviors, Autism, Acquired Brain Injury (ABI), mental health conditions.

We are looking for experienced professionals who have a minimum of 1+ years of experience in disability support, specifically working with Complex Care and challenging behaviors. You will provide daily living tasks, personal care, and medication support to our clients. You'll also assist with bowel care, stoma care, PEG feeding & urinary catheter support, and implement positive behavior support strategies to manage complex behaviors.

In this role, you will work closely with individuals in their own homes to enhance their independence, quality of life, and community participation. Your responsibilities will include:

  • Supporting with daily living tasks, personal care, and medication support
  • Assisting with bowel care, stoma care, PEG feeding & urinary catheter support
  • Providing transportation for appointments, shopping, and social activities
  • Implementing positive behavior support strategies to manage complex behaviors
  • Supporting clients in achieving their NDIS goals, promoting inclusion and empowerment
