Primary Healthcare Triage Specialist

We are seeking a skilled Primary Healthcare Triage Specialist to join our team at Healthcare Australia. As a key member of our virtual healthcare setting, you will play a vital role in providing timely and appropriate care to patients.
About the Role:
- Conduct comprehensive virtual assessments to evaluate patient health concerns, including acute and chronic conditions.
- Prioritize cases based on symptom severity and urgency, ensuring seamless coordination of care with primary care physicians and other healthcare providers.
- Collaborate with multidisciplinary teams to develop personalized care plans and provide follow-up support to patients.
- Refer patients to specialized care, diagnostics, or follow-up services as needed, utilizing telehealth platforms for efficient delivery of care.
- Provide education on self-care, medication management, and preventive health measures to promote patient empowerment.
What You'll Need:
- A Bachelor's degree in nursing with current AHPRA registration.
- A minimum of 5 years of experience in clinical program management, preferably in primary healthcare triage or case management.
- Strong proficiency in virtual care delivery, including telehealth platforms and electronic health record systems.
