
Homeless Support Coordinator

Job Summary:
We are seeking a skilled and compassionate professional to join our team as a Case Manager.
The successful applicant will provide assertive outreach to individuals and families who are homeless or at risk of becoming homeless, enhancing their quality of life and status in the community by linking them to secure housing options.
About the RoleThis is an exciting opportunity for someone who is passionate about delivering high-quality support services and making a real difference in people's lives.
The Case Manager will work closely with program participants, developing early intervention strategies to achieve both program and individual goals. They will conduct initial assessments to ensure program eligibility, using tools like the Outcome Star to track progress and identify barriers to achieving goals.
The successful applicant will also be responsible for maintaining accurate records of participant information using electronic data systems, adhering to service agreements, legislation, and program policies.
In addition, they will develop and maintain community partnerships through networking, referrals, and promotion of Centacare NQ's programs.
Key Responsibilities:

Requirements:
- Degree-level qualification in Social Work, Human Services, or Behavioural Sciences, or a Certificate IV or higher qualification in the community sector with relevant experience.
- Excellent written and verbal communication skills, strong analytical skills, and a current driver's licence.
- A Disability Worker Screening and a Working with Children Blue Card (or eligibility to obtain).
