The Clinical Case Manager position plays a pivotal role in supporting the delivery of integrated and coordinated resident care services. This includes pre-entry clinical assessments, move-in support, care planning, care delivery, evaluation, and referral to externally funded care with associated documentation.
As a key member of our multidisciplinary team, you will be responsible for ensuring timely and effective clinical assessments of Home Care Package recipients, DVA, CHSP, or Privately funded residents. Your primary goal is to undertake or oversee care and support requirements of residents to determine their care needs and plan accordingly.
Key aspects of this role include:
- Supporting a coordinated care process for residents within your portfolio
- Providing timely and effective clinical assessment of Home Care Package recipients, DVA, CHSP, or Privately funded residents
- Undertaking or overseeing care and support requirements of residents to determine their care needs and plan accordingly
- AHPRA registration (Registered Nurse)
- Membership of a professional body (Desirable)
Experience in the community care sector and aged care nursing is desirable. The ideal candidate will possess strong commitment to delivering high-quality care and customer service while working as a member of a multidisciplinary team.
