Case Manager Job Description

  • Job Summary:

A Case Manager provides patient care coordination and assessment of patients' level of care needs, ensuring seamless transitions and optimal quality, cost, and service outcomes. This role requires effective communication with patients, physicians, and nurses to identify post-discharge services needed.

  • Key Responsibilities:
  • Assess and evaluate patients' discharge planning needs and document timely in the patient's medical record.
  • Communicate with patients, physicians, and nurses to identify post-discharge services needed.
  • Identify issues that may delay patient discharge and facilitate timely resolution of these issues.
  • Develop and document a post-discharge care plan in conjunction with other members of the healthcare team.
  • Coordinate care post-discharge with physician, insurance carrier, post-acute providers, patient, and family.
  • Provide medical information to ensure smooth transitions of care.
  • Collect data about patient progress and modify discharge plans based on changes in patients' plan of care.
  • Complete all documents required by payors and regulatory agencies timely and accurately.
  • Assist with orientation of new personnel.
  • Complete timely utilization review (level of care criteria reviews and insurance reviews).
  • Hospital Expectations:
  • Adhere to organization-wide and other applicable policies and procedures.
  • Day-to-day performance complies with the hospital's Service Excellence Behavioral Expectations.
  • Perform within the prescribed limits of the hospital's Ethics, Compliance, and Confidentiality Program guidelines.
  • Perform within professional scope of practice at all times.

Required Skills and Qualifications:

  • Associate's degree in a relevant field.
  • Hospital or healthcare experience.
  • Effective verbal and written communication skills.
  • Direct patient care experience or Social Worker experience.

Preferred Qualifications:

  • Degree in Social Work or Nursing.
  • Bachelor of Science degree in Nursing.
  • 1-2 years of hospital case management experience.
  • Licenses/Certifications:
  • RN, LVN, or LSW licensure/certification.
  • LMSW, CPCM, CPUR, CCM certifications.
