The Role

As a Clinical Coordinator, you will assess, coordinate and manage HITH patient admissions under a care coordination model.

Your key responsibilities will include promoting the HITH service within the organization and to external stakeholders. You will support efficiency and innovative ways of working with the electronic medical record system within HITH to ensure excellence in patient care.

This role requires autonomous decision making and demonstrated leadership. You will model leadership, direction and support to the HITH Nurse Clinicians and work with the Nurse Manager and Clinical Nurse Manager to manage a portfolio or quality project.
