We are seeking an experienced and motivated Registered Nurse to join our team as a Post-Acute Care Coordinator. This role involves supporting seamless transitions from hospital to home through coordinated, patient-centered care.

Echuca Regional Health provides a comprehensive range of acute, medical, surgical, and sub-acute services, as well as residential aged care and community healthcare services. The ideal candidate will have current AHPRA registration as a Registered Nurse, comprehensive knowledge of discharge planning processes, and strong leadership and collaboration skills.

Key Responsibilities:

  • Develop flexible and tailored care packages.
  • Actively participate in mandatory meetings as required.
