We are seeking an experienced and motivated Registered Nurse to join our team as a Post-Acute Care Coordinator. This role involves supporting seamless transitions from hospital to home through coordinated, patient-centered care.
Echuca Regional Health provides a comprehensive range of acute, medical, surgical, and sub-acute services, as well as residential aged care and community healthcare services. Our focus is on increasing capacity as a sub-regional teaching health service, with employees dedicated to delivering the best possible care to their community.
The ideal candidate will have current AHPRA registration as a Registered Nurse, comprehensive knowledge of discharge planning processes, and strong leadership and collaboration skills.
Key Responsibilities:- Coordinate care by negotiating, purchasing, and monitoring community-based supports from contracted providers.
