Clinical Aged Care Specialist

The Mid North Coast region is currently seeking a skilled Clinical Aged Care Specialist to join their team, providing assessments and referrals for Older Australians.
This part-time position involves working three days a week with flexible hours, offering a supportive work environment and opportunities for growth and development.
Key Responsibilities:
- Collaborate with clients, families, and multidisciplinary teams as required;
- Work collaboratively with colleagues to promote a positive team culture;
The successful candidate will use the Integrated Assessment Tool (IAT) to identify client needs and determine eligibility for home support services. They will also review support plans to meet changing client needs.
Requirements:
- Tertiary qualifications in a health-related discipline;
- Current unrestricted registration with the Australian Health Practitioners Regulation Agency (AHPRA);
- Prior experience in Aged Care settings or working with Older Australians is essential.
