Clinical Aged Care Professional
">About the Role:
This is an exciting opportunity for a qualified and experienced Clinical Aged Care Professional to join our team. As a key member of our multidisciplinary team, you will be responsible for conducting comprehensive assessments of clients to identify their complex needs and determining eligibility for aged care services under the Aged Care Act.
The ideal candidate will have a tertiary qualification in a health-related discipline directly related to health, aged care or a related specialist area, current unrestricted registration with the Australian Health Practitioners Regulation Agency (AHPRA), an Australian driver's license, and access to a registered, insured vehicle.
You will work collaboratively with clients, families, and multidisciplinary teams as required, conducting accurate assessments using the Integrated Assessment Tool (IAT) to determine client needs and eligibility for home support services.
This role involves reviewing support plans to meet clients' changing support needs, working effectively both autonomously and as a member of a broader team in delivering to key performance indicators within identified timeframes.
Maintaining currency of knowledge relating to aged care services, both funded and non-funded, is essential for this role. You will also assist with continuous improvement frameworks to regularly evaluate and enhance assessment processes and outcomes.
Requirements:- Tertiary Qualification – in a health-related discipline directly related to Health, Aged Care or related specialist area (e.g. Registered Nurse, Medical Officer, Occupational Therapist, Physiotherapist or Social Worker)
- Current unrestricted registration with the Australian Health Practitioners Regulation Agency (AHPRA)
- An Australian Driver's License and access to a registered, insured vehicle
- Minimum of one year demonstrated experience in Australia or Overseas directly delivering services in Aged Care settings and/or to Aged persons including to people living with dementia
