Mission Australia is committed to ending homelessness and ensuring people in need can thrive. As a Support Coordinator, you will play a vital role in providing holistic support to individuals in our Housing and Support program.

Your key responsibilities will include:

  • To respond to referrals to the service, conduct thorough assessments, and establish case plans with clients and relevant stakeholders.
  • To provide evidence-informed case management sessions with clients, aligned with Mission Australia's National Case Management Approach.
  • To contribute to the development and facilitation of group sessions that meet the needs of the service, clients, and local community.
  • To develop, implement, and review individualized case plans for each client, addressing issues such as living skills, welfare needs, interpersonal skills, health, education, training, employment options, social skills, recreational needs, and family relationships.
  • To establish records, monitor, and review each person's progress in consultation with the Program Manager.
  • To develop quality relationships with key stakeholders, including community organizations, rehabilitation providers, and other health professionals, to ensure effective provision of support and information to clients.

You will work closely with a diverse range of clients who may be experiencing complex issues, including homelessness, domestic violence, poverty, abuse, mental health, and substance misuse. Your ability to provide specialized or complex therapeutic interventions and manage a caseload of complex consumers is essential.

We are seeking an experienced Support Coordinator with a degree or diploma and at least 1 year of case management experience. You will have a strong understanding of social and/or affordable housing, as well as the ability to develop high-quality relationships with stakeholders.

The successful candidate will possess excellent communication, problem-solving, and time-management skills, with the ability to work independently and as part of a team. They will be passionate about making a real difference in the lives of Australians in need and committed to delivering exceptional results.
