
Supportive Care Specialist

This role involves supporting individuals with brain injuries and their families through social work services in a multidisciplinary team, working across home, community and centre-based settings.
Providing psychosocial assessment and counselling to support individuals with brain injuries and their families.
Collaborating with allied health professionals, including occupational therapists, psychologists and speech pathologists, to develop and implement comprehensive treatment plans.
Liaising with external care providers, such as hospitals, rehabilitation centres and community organisations, to ensure seamless care coordination and transition planning.
Maintaining accurate and detailed clinical records and reports, ensuring compliance with relevant legislative and regulatory requirements.
Supervising students and contributing to service planning, development and evaluation, ensuring alignment with organisational goals and objectives.
Key responsibilities include:
Developing and implementing individualised rehabilitation plans, incorporating person-centred approaches and strengths-based practice.
Conducting regular assessments and reviews, using evidence-based tools and frameworks, to inform treatment decisions and monitor progress.
Building and maintaining relationships with clients, families, carers and other stakeholders, fostering a collaborative and supportive environment.
Participating in multidisciplinary teams, sharing knowledge, expertise and best practices to enhance service delivery and outcomes.
Staying up-to-date with latest research, trends and developments in the field of brain injury rehabilitation, applying this knowledge to improve practice and inform policy.
Requirements for success in this role include:
Demonstrated advanced knowledge, skills and experience in social work assessment and intervention, within a specialist rehabilitation or mental health setting.
Strong understanding of community-based brain injury rehabilitation, including current models of practice and policy.
Experience working in strengths-based practice, family and person-centred approaches, and multidisciplinary teams.
Excellent time management, written and verbal communication, report writing and negotiation skills.
Ability to interact constructively and collaboratively with diverse stakeholders, including people with brain injuries, families, carers and healthcare professionals.
