
Senior Veteran Care Specialist

The role is a temporary contract to start ASAP on an ongoing basis, working Monday to Friday from 9am to 5pm.
You will be responsible for providing veteran-centric, trauma informed case management practice to a high-quality standard to veterans and their families, managing caseloads, being knowledgeable when relaying details about contact and program information, providing support in how to navigate DVA business accessing legislative entitlements, external services and clinical support access to other Case Managers.
Key Responsibilities:
- Provide point of contact details to clients and coordinate access to services
- Provide an escalation point for clients at serious risk of harm to self or others
- Provide specialised case management and/or clinical expertise
- Manage a limited case load of highly complex clients
- Assist clients to access support services for their psychological and physical health, and medical needs
- Preparation of client summaries and briefs
- Undertake client assessments, intake assessments and risk assessments
- Escalation of high risk and profile issues through leadership as required
- Provide clinical advice and coaching support to Client Support Program colleagues and other DVA staff, as required
Requirements:
- Must have worked in a similar position as a senior case manager
- Experience working within a mental health or community case management setting, including working with people who present with complex mental health conditions is essential
- Experience in coordinating and assisting in the management of a caseload of complex and potentially at-risk clients in community mental health teams will be ideal
- Tertiary qualifications (Bachelor's degree or higher) in a relevant health field including Nursing, Psychology, Occupational Therapy, or Social Work
- Excellent communication, organisational skills and the ability to handle complex situations and work well under pressure
- Proficient in all Microsoft Office packages
