Job Title: Veterans Home Care Team Leader

We are seeking a highly skilled and compassionate individual to join our team as a Veterans Home Care Team Leader. In this role, you will play a critical part in delivering high-quality care to veterans who need it most.

About the Role:

  • Conduct thorough assessments of veterans' needs using our standard assessment tool.\
  • Accurately and efficiently document all interactions with veterans.\
  • Coordinate follow-up referrals and service reviews to ensure seamless delivery of care.\
  • Maintain up-to-date knowledge of community-based health and support services to provide informed guidance to veterans.\
  • Contribute to quality improvement initiatives to enhance the overall care experience.\
  • This is a full-time, fixed-term position to cover maternity leave. You will work from our office in Ringwood, Victoria, but serve eligible veterans in Western Australia during Western Australian business hours.\
  • The ideal candidate will have a minimum of two years' experience in the aged or community care sector and possess comprehensive assessment and care planning skills.\
  • You will be passionate about creating a positive and supportive workplace culture that values diversity and inclusion.\
