Assessing Veteran Care Needs

This role offers a supportive working environment, salary packaging benefits, and flexible start times to accommodate child care schedules.

We are seeking an experienced professional to play a crucial role in enhancing the experience of eligible members of the Veteran community through empathy, warmth, understanding, and kindness. You will engage in meaningful work, providing exceptional customer service and have the opportunity to make a difference in the lives of those you care for.

Key Responsibilities:

  • Understanding program parameters and communicating this to clients, families, service providers, and health professionals;
  • Managing inbound and outbound calls from Veterans, their families/representatives, service providers, and relevant government departments;
  • Completing assessments and reviews to identify special needs, specific goals, or desired outcomes to maximise independence;
  • Recommending appropriate services and other primary health, allied health, or aged and community care services;
  • Accurately documenting all communication in relevant programs.

Requirements:

  • Experience in assessment, care planning, and service coordination;
  • Minimum 2 years of experience in the aged or community care sector;
  • Experience as a Personal Care Assistant (PCA) or Personal Care Worker (PCW) (desirable);
  • Excellent customer service skills, including a professional and courteous manner and a collaborative 'can-do' approach (desirable);
  • Knowledge of the veteran community and the impact of military training/experiences on client's physical and mental health (desirable);
  • Proficiency in Microsoft Office programs;
  • A genuine desire to promote people to live at home;
  • Ability to maintain confidentiality;
  • Excellent verbal and written communication skills.
