Responsible in determining eligibility and assess the needs of individual veterans for home care services in a timely way, using the phone based Veteran's Home Care (VHC) standard assessment instrument; complete relevant documentation; arranging follow-up referrals and review of services; maintaining knowledge of community based health and support providers and participation in quality activities.

This role delivers a phone based service for eligible Veterans located in Western Australia from our offices located in Victoria. Hours of work for this role will be Western Australian business hours.

**To be successful, you will require;**
- Minimum two years' experience in the aged or community care sector (desirable but not essential)
- Comprehensive assessment and care planning skills
