Empower Lives as a Care Coordinator

We are seeking a dedicated professional to join our team in empowering clients to live as independently as they choose. As a key member of our Client Services Team, you will play a vital role in enabling clients to make informed decisions about their care and services.

Key Responsibilities:
  • Develop and oversee personalized care plans, risk assessments, and service agreements to ensure client-centered outcomes.
  • Mentor, support, and supervise a team of Support Workers to deliver high-quality care services.
  • Conduct comprehensive client onboarding, home visits, and regular check-ins to monitor progress and address concerns.
  • Collaborate with clients, families, and stakeholders to foster strong relationships and achieve person-centered goals.
  • Manage client documentation, incidents, complaints, and continuous improvement processes to maintain a safe and supportive environment.
Requirements:
  • Minimum 2 years' experience in care coordination or case management with a focus on empowering clients to live independently.
  • Strong knowledge of disability support, complex care, and mental health principles.
  • Proven leadership and staff supervision skills with a track record of delivering high-quality care services.
  • Excellent communication, organization, and time management skills with the ability to work effectively in a fast-paced environment.
  • Current Working with Children Check, NDIS Worker Screening, Police Check, Driver's Licence, and access to own vehicle.
  • Relevant tertiary qualification in Disability, Allied Health, Social Science or similar (highly regarded).
