Job Description

We are seeking a skilled professional to fill the role of Case Manager in our Home Health department.

The ideal candidate will have expertise in managing client cases, determining client goals and needs, and providing continuity of care across the continuum.

Key Responsibilities:
  • Manage a caseload of clients requiring supportive case management skills.
  • Determine client goals and needs, assess client situations, work effectively with community resources, and plan for care and support based on client health needs.
  • Provide continuity of care by assessing clients, liaising with team members, coordinating clinical interventions, evaluating client outcomes based on client goals for care, coordinating ongoing care, and supporting transition.
  • Act as a clinical resource for the interdisciplinary team by providing information based on specialized theory and practice related to clients with complex healthcare needs.
Required Skills and Qualifications

To be successful in this role, you will require:

  • A current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
  • Three years' recent clinical nursing experience in Home Health and case management or an equivalent combination of education, training, and experience.
