Job Opportunity

Key Responsibilities:

  • Assess client needs and develop individualized care plans
  • Provide ongoing support and guidance to clients and their families
  • Collaborate with other healthcare professionals to ensure comprehensive care
  • Monitor client progress and adjust care plans as needed

Requirements:

  • Degree in Nursing or related field
  • Current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM)
  • Three years' recent clinical nursing experience in Home Health and case management or an equivalent combination of education, training and experience
