Support Worker: Delivering Personalized Support Services

We are seeking a skilled and dedicated individual to join our team as a Support Worker.

Job Description

The role of a Support Worker is to provide personalized support services to seniors and individuals with disabilities, enabling them to live independently in their homes and communities.

Key Responsibilities:
  • Providing tailored person-centered services and activities that meet the unique needs and preferences of each customer.
  • Working closely with customers, their families, carers, community agencies, and other professionals to ensure seamless support delivery.
  • Assisting with daily living tasks, such as domestic assistance, and promoting independence and self-sufficiency.
  • Developing and maintaining professional relationships with customers and stakeholders.
  • Collaborating with the support team to ensure consistent and high-quality service delivery.
Required Skills and Qualifications:
  • Certificate III Individual Support - Aged Care or Disability, or other relevant qualification and/or experience.
  • Experience working with seniors, people on the mental health recovery journey, and those living with disability.
  • Excellent communication skills to interact with diverse range of customers and stakeholders.
  • Ability to work independently and as part of a team.
  • Good written communication, computer, and mobile device skills.
