We are seeking a skilled and dedicated individual to join our team as a Support Worker.
Job DescriptionThe role of a Support Worker is to provide personalized support services to seniors and individuals with disabilities, enabling them to live independently in their homes and communities.
Key Responsibilities:- Providing tailored person-centered services and activities that meet the unique needs and preferences of each customer.
- Working closely with customers, their families, carers, community agencies, and other professionals to ensure seamless support delivery.
- Assisting with daily living tasks, such as domestic assistance, and promoting independence and self-sufficiency.
- Developing and maintaining professional relationships with customers and stakeholders.
- Collaborating with the support team to ensure consistent and high-quality service delivery.
- Certificate III Individual Support - Aged Care or Disability, or other relevant qualification and/or experience.
- Experience working with seniors, people on the mental health recovery journey, and those living with disability.
- Excellent communication skills to interact with diverse range of customers and stakeholders.
- Ability to work independently and as part of a team.
- Good written communication, computer, and mobile device skills.

- NDIS Worker Screening Check (or willingness to apply for one prior to commencing employment).
