
Care Coordinator for Vulnerable Families

The Pregnancy Family Meetings (PFMs) program is a collaborative initiative between government agencies to promote early engagement and interagency planning with pregnant women at risk of their babies entering out-of-home care at birth.
As a key facilitator position in the PFM Program, you will work directly with families and agencies to schedule and facilitate PFMs, supporting safe outcomes for babies through early collaboration.
- Main Responsibilities:
- Facilitate Pregnancy Family Meetings to promote early engagement and interagency planning with pregnant women at risk of their babies entering out-of-home care at birth.
- Develop and maintain a group of appropriately skilled PFM Facilitators from within the local community.
- Foster strong partnerships to sustain and further develop the PFM Program.
- Essential Criteria:
- Current registration with the relevant health practitioner regulatory body.
- Evidence of diversity claim if applicable.
- Selection Criteria:
- Relevant tertiary qualifications in social work or counseling and eligibility for membership of the relevant professional association; or relevant tertiary qualification in psychology and registration.
- Experience in delivering health services to at-risk and vulnerable children, young people, and their families.
- Understanding of child protection policy and practice and the needs of children and young people in out-of-home care.
- Demonstrated ability to work collaboratively with other professionals both internally and with external partners.
- High-level written and verbal communication skills, including report writing skills and computer skills.
The ideal candidate will possess excellent interpersonal and organizational skills, with the ability to work effectively in a team environment and manage multiple priorities. They will also have strong analytical and problem-solving skills, with the ability to think critically and make sound judgments.
