Hammond Care provides care primarily through hospitals, residential care homes and home-based care as well as services for dementia support, community palliative care, rehabilitation, positive aging and older persons' mental health.
About the role The Pastoral Care Coordinator provides pastoral care, leadership and support to clients, their families, team members and pastoral care volunteers.
The role supports clients by visiting them in their own homes across the Central Coast.
The role requires access to a car (mileage reimbursed) and is permanent part-time working 4 days per week (30.4 hours pw) based out of our Tuggerah office.

Participating in opportunities such as staff meetings, in-services, expos, and other events with the purpose of providing information about pastoral care opportunities and creating partnerships.
Regularly meeting with local service managers and team members for the purpose of receiving feedback, identifying need, and reporting back on any changes or developments in the Pastoral Care service.
