Job Title: Case Manager Tsil. Permanent Full Time

We are seeking a skilled and compassionate Care Coordinator to join our team.

This is an excellent opportunity for an experienced professional to make a positive impact in the lives of children and young people.

About the Role:

  • Develop and implement high-quality therapeutic case plans for children and young people in care.
  • Work closely with the team to support the achievement of case plan goals.
  • Collaborate with local community services to provide a comprehensive support network.

Key Responsibilities:

  • Develop leaving care plans for each young person over 16 to support their transition to independent living.
  • Drive the implementation of Leaving care Plans / P4I Plans / Cultural Support Plans.
  • Actively engage children, young people, their families, and link them in with local community services.

Requirements:

  • A relevant bachelor's degree or a relevant diploma and be working towards a degree.
  • Previous case management experience within an Intensive Therapeutic Care or Out Of Home Care (OOHC) setting.
