Nurse for Vulnerable Families

This role is a vital part of our team, working with children and families to provide them with the care and support they need.

As a Nurse for Vulnerable Families, you will be responsible for providing high-quality patient care to individuals who are vulnerable due to various factors such as poverty, homelessness, or lack of access to healthcare. You will work closely with other healthcare professionals to develop and implement individualized care plans that address the unique needs of each patient.

  • Key Responsibilities:
  • Provide high-quality patient care to individuals who are vulnerable due to various factors
  • Work closely with other healthcare professionals to develop and implement individualized care plans
  • Conduct thorough assessments of patients' physical and emotional needs
  • Develop and implement personalized care plans that address the unique needs of each patient
  • Monitor patients' progress and adjust care plans as needed
  • Collaborate with other healthcare professionals to ensure comprehensive care

Requirements:

  • Current Authority to Practice as a Registered Nurse
  • Child and Family Health nursing qualifications or commitment to undertake and complete in first 12 months
  • Demonstrated recent experience as a Registered Nurse in an environment appropriate to the Child and Family Health Nursing Speciality
  • Demonstrated ability to provide safe compassionate quality care that is culturally responsive
  • Demonstrated ability to effectively communicate and engage with clients/consumers, carers and the multidisciplinary health care team
  • Demonstrate evidence of the use of critical thinking as it relates to the delivery of evidenced based nursing care
  • Possess an Australian C-Class driver's license (including P1 or P2)
