Homelessness Support Specialist

The Salvation Army
is seeking a values-driven and proactive Assertive Outreach Worker to work as part of a team with people experiencing primary homelessness, rough or street sleeping, and at risk of primary homelessness, to develop and implement case plans that will facilitate their transition to supported accommodation or independent living.
The successful candidate will have a tertiary qualification in social work, community welfare, community development or a related field, and experience and expertise in the areas of Primary Homelessness, Poverty, Social isolation, Domestic Violence, Mental Health, general health, people exiting prison, Disabilities, Youth, Legal issues and AOD.
Key Responsibilities- Establish effective relationships with rough and street sleepers, and people at risk of primary homelessness to encourage self-determination and social and economic participation.
- Provide assertive outreach and support to clients.
- Provide ongoing assessment of clients' housing and health issues, needs and circumstances from initial intake to exit.
- Develop individual case plans in collaboration with clients that includes a range of interventions that address the needs and goals of clients.
- Assist clients to access appropriate support services and establish community supports, and where needed ensuring clients are assisted through advocacy with service providers.
