About the Role

We are seeking a highly skilled and compassionate Assertive Outreach Worker to join our team in Launceston, Tasmania. As a key member of our Homelessness Stream, you will work closely with individuals experiencing primary homelessness, rough sleeping, and at risk of primary homelessness to develop and implement case plans that facilitate their transition to supported accommodation or independent living.

Key Responsibilities
  • Establish effective relationships with clients to encourage self-determination and social and economic participation.
  • Provide assertive outreach and support to clients, including ongoing assessment of their housing and health issues, needs, and circumstances.
  • Develop individual case plans in collaboration with clients, incorporating a range of interventions to address their needs and goals.
  • Assist clients in accessing appropriate support services and establishing community supports, and advocate on their behalf when necessary.
Requirements
  • A tertiary qualification in social work, community welfare, community development, or a related field.
  • A current Working with Vulnerable People check.
  • A current Tasmanian Driver Licence.
  • Experience and expertise in the areas of primary homelessness, poverty, social isolation, domestic violence, mental health, general health, people exiting prison, disabilities, youth, legal issues, and AOD.
