About the Role

We are seeking a dedicated and proactive Social Worker to join our team in Launceston, Tasmania. As an Assertive Outreach Worker, you will work with individuals experiencing primary homelessness, rough or street sleeping, and at risk of primary homelessness.

Reporting to the Manager, this is a permanent, part-time (30.4 hours per week) position based in Launceston, Tasmania. The successful candidate will be responsible for developing and implementing case plans that facilitate clients' transition to supported accommodation or independent living.

Your Responsibilities
  • Work as part of a team with the support of other Street to Home Outreach workers.
  • Establish effective relationships with clients to encourage self-determination and social and economic participation.
  • Provide assertive outreach and support to clients.
  • Conduct ongoing assessments of clients' housing and health issues, needs, and circumstances from initial intake to exit.
  • Develop individual case plans in collaboration with clients that include a range of interventions addressing their needs and goals.
  • Assist clients in accessing appropriate support services and establishing community supports, ensuring they receive advocacy with service providers when needed.
Your Requirements
  • A tertiary qualification in social work, community welfare, community development, or a related field.
  • A current Working with Vulnerable People check.
  • A current Tasmanian Driver Licence.
  • Experience and expertise in areas such as primary homelessness, poverty, social isolation, domestic violence, mental health, general health, people exiting prison, disabilities, youth, legal issues, and AOD.
About Us

The Salvation Army Australia's Homelessness Stream provides high-quality, person-centred services to individuals at risk or experiencing homelessness. We support adults and families, including accompanying children, with accommodation, case management, outreach support, financial assistance, connection, and referral to other specialist services.

The 'Street to Home' program offers flexible support to people who are rough or street sleeping and at risk of primary homelessness.

The estimated annual salary for this role is approximately $65,000-$75,000 depending on experience, with the opportunity to access additional benefits and bonuses.
