Care Coordinator for Individuals with Disabilities

Sunflower Community Supports is an NDIS provider seeking a passionate and dedicated individual to improve lives of valued participants.
**About the Role**
We are looking for someone who can provide 2:1 care for NDIS participants with high intensity disabilities. As a Care Coordinator, you will work in the disability sector providing support that enables individuals to lead fulfilling lives.
Your key responsibilities include:
- Manual Handling/using hoist
- Changing incontinence products
- Assisting participants with daily personal hygiene and dressing tasks
- Performing home based tasks such as food preparation and housework
- Providing emotional support and friendship
- Encouraging Independence
- Assist Participants to access the community safely
- Follow all policies and procedures around providing person centered care
**Requirements**
To be successful in this role, you will require previous experience as a Support Worker, good communication skills and commitment to upholding our values. You will also need:
- Covid-19 Vaccination
- NDIS worker screening clearance
- First Aid Certificate
- Current Driver's Licence and access to a reliable vehicle
