
Care Coordinator for Individuals with Disabilities

- Work closely with participants to effectively utilise their NDIS-funded supports and connect them with appropriate service providers.
- Coordinate and monitor progress towards achieving plan objectives, promoting inclusivity and informed decision-making.
- Ethical customer-focused practice to provide ongoing support and guidance to participants and their support network.
- IDentify and coordinate service providers and supports to ensure goals are achieved and services delivered as per the participant's plan.

About YouYou are a highly motivated individual with excellent knowledge of the NDIS system and processes, exceptional verbal and written communication skills, and high-level organisational abilities. To be successful in this role, you must possess:
- A solid understanding of the National Disability Insurance Scheme (NDIS) and specifically the function of NDIS Support Coordination.
- Experience in utilising community-based supports and methods to achieve participant goals.
- High-level knowledge and skills in coordination of support for individuals with a disability in their community or home.
- A solid grasp of quoting, budgeting, and financial procedures with strong financial acumen.
A NDIS Workers Screening Check and Working with Children Check are mandatory requirements for this role. If you do not already hold these, you will be required to obtain them prior to employment.
