
Care Coordinator for Individuals with Disabilities

We are seeking a dedicated Care Coordinator to provide support to individuals with disabilities in their homes and community across Victoria.
About the Role:- The successful candidate will be responsible for providing day-to-day living assistance, including support with daily activities such as meal preparation, dressing, bathing, and medication administration.
- They will also assist individuals in setting routines and building capacity for daily living skills, while promoting physical, social, emotional, and intellectual development.
- A minimum of Cert III or Cert IV in Disability or equivalent previous experience in the disability field is essential.
- Fluency in Cantonese is required, and a valid NDIS Worker Screening Check, First Aid Certificate, and CPR certification are mandatory.
- Working with Children's Check, driver's license, vaccination proof, and ability to work independently as part of a team are also required.
