We are seeking a highly skilled Clinical Pharmacist to join our team and contribute to optimizing medication use and improving health outcomes for our patients.

About the Role

This position will work closely with interdisciplinary teams to develop and implement population-based care plans, ensuring that medications are used safely and effectively. The successful candidate will have a strong analytical mind and excellent communication skills, with the ability to interpret data and communicate findings effectively.

Key Responsibilities:
  • Develop and Implement Care Plans: Work with healthcare professionals to identify opportunities for improvement and develop strategies for implementation.
  • Analyze Data: Use data to inform medication use and identify trends or areas for improvement.
  • Communicate Effectively: Communicate effectively with patients, families, and healthcare providers to ensure seamless transitions of care.
Requirements:
  • A Doctor of Pharmacy degree from an accredited institution.
  • Current licensure as a registered pharmacist in the state of Ohio.
  • Experience working in a clinical setting, preferably in a hospital or ambulatory care environment.
  • Strong analytical and problem-solving skills, with the ability to interpret data and communicate findings effectively.
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse populations.
