Summary
In partnership with the SHMG Director of Outcomes and Advanced Payment Models and Chief Medical Officer of SHMG, leads the development and implementation of SHMG medication management and supports value-based care quality activities across the medical group, and provides direct support for the care coordination and transition planning for SHMG patients. The Population Health Pharmacist will play a key role in optimizing medication related outcomes for patients within the medical group and manage medication therapy opportunities across all populations served by the medical group. Provides program development support for transitional medication management and support services for the Summa Health Medical Group.
Minimum Qualifications
- Formal Education Required:
- Graduate of an accredited school of Pharmacy and licensed in the State of Ohio.
- PGY-1 Residency or Equivalent Clinical Experience Preferred
- Experience and Training Required:
- Experience in pharmacy practice, preferably in ambulatory setting
- Experience in program development, implementation, and evaluation;
- Training or experience in population health concepts and practices;
- Experience in working in and leading interdisciplinary teams to improve patient outcomes.
- Other Skills, Competencies and Qualifications:
- Population Specific Competency: Ability to effectively interact with patients/customers with the understanding of their needs for self-respect and dignity.
- Provide evidence-based pharmacotherapy recommendations to support chronic disease management.
- Ability to appropriately identify patients using risk stratification metrics/programs.
- Collaborates with providers, care managers, and other clinical staff to provide recommendations that improve outcomes and reduce medication-related risks.
- Ability to coach patients to optimal self-management through motivational coaching and support medication adherence and transitions of care by reviewing discharge medications.
- Develop clinical pathways and protocols to support population health goals.
- Ability to prioritize duties and delegate responsibilities.
- Ability to utilize technology to effectively communicate with patients and healthcare providers in a virtual environment.
- Participate in payer driven quality programs (e.g.: HEDIS, ACO metrics)
- Level of Physical Demands:
- Sedentary: Exerts up to ten pounds of force occasionally and/or a negligible amount of force frequently
- Hazards: Exposure to blood, blood fluids, and sharps; risk of falls, strains, contagious diseases, cuts from broken glass, needle puncture, etc. Possible exposure to selected hazardous substances (e.g., alcohol, chemotherapeutic agents, etc.)
$54.22/hr - $81.33/hr
The salary range on this job posting/advertising is base salary exclusive of any bonuses or differentials. Many factors, such as years of relevant experience and geographical location are considered when determining the starting rate of pay. We believe in the importance of pay equity and consider internal equity of our current team members when determining offers. Please keep in mind that the range that is listed is the full base salary range. Hiring at the maximum of the range would not be typical.
