The Clinical Care Manager plays a vital role in overseeing the delivery of high-quality person-centered care, ensuring that residents receive the best possible attention and support.
This position requires strong leadership and coordination skills, with the ability to work collaboratively with a multidisciplinary team to achieve shared goals and objectives.
The Clinical Care Manager is responsible for providing clinical oversight and guidance to nursing staff, ensuring that they have the necessary resources and support to deliver exceptional care to residents.
Key responsibilities include:
- Providing clinical leadership and coordination to ensure high-quality care is delivered to all residents living in the facility.
- Examining and reviewing clinical incidents, audit data, and clinical indicators to identify areas for improvement and develop strategies to address them.
- Facilitating regular meetings with the nursing team and Facility Manager to discuss incident analysis and reporting.
- Providing guidance and oversight on the High-Risk Resident Framework to ensure residents who are identified as having high-risk needs are supported and cared for using a multi-disciplinary approach.
- Ensuring compliance with Aged Care Quality Standards and maintaining the highest standards of privacy and confidentiality at all times.
Required Skills and Qualifications
To be successful in this role, you will need:
- A minimum of five years clinical experience with a focus on person-centered care and a commitment to the needs of older people.
- Advanced knowledge and experience in dementia care, clinical care coordination, including chronic and complex health challenges, and aged care planning.
- Comprehensive understanding of AN-ACC assessments and the ability to manage and coordinate these processes.
- Well-developed communication and interpersonal skills, including the capacity to work cooperatively within a team environment, effectively negotiate, resolve conflict, and provide advice/guidance to staff.
- Capacity to manage in a pressure environment and exercise sound judgment in complex situations while working collaboratively in a multidisciplinary team.
